The heroes encounter a medusa and her mate, a maedar. They occupy a cave complex near Helmsley and have been at war with the grell necromancers the PCs had previously encountered nearby.

The cave complex is guarded by a number of grimlock, servants of the medusa.

At the heart of the cave complex is a shrine with an altar in the form of huge stone medusa head dedicated to the evil goddess Hel. A secret passage via a well hidden secret door in this chamber leads to a deeper series of passages and chambers which the heroes explored later in Beneath the Medusa's Lair.
